Self-Assessment Questionnaire 

There are four self-assessment tools in this package. Three of these assess the success of the implementation of the HSE-MS and the fourth assesses the implementation of sustainable development (SD).

Please read the following information prior to answering a self-assessment questionnaire.  If clarification is still required, please contact MSE/4 regarding the HSE-MS SAQs or MSE/6 regarding the SD SAQs.

Purpose

The self-assessment tool will help assess the level of alignment of company business processes with the HSE Management System (HSE-MS) / sustainable development and to identify where there are gaps. The prime objective is to identify actions that should lead to change and improvement of alignment within PDO.

By aggregating the results of different assets / units across the Company, a measure of overall progress is provided and common shortfalls requiring corporate action are identified.  The prime aim is ‘improvement’, therefore honesty in completion is essential. The assessment scores themselves are less important than the underlying messages of ‘Where are we scoring low?  Why?  Where will increased focus be most effective?’.

As the self-assessment questionnaire is subjective and personal, it is advised that the system is not used as the basis for employee or contractor performance related reward schemes.

Structure of the Self-Assessment Questionnaire

The self-assessment questionnaire comprises a series of short statements describing the degree of alignment at four levels – from minimal (Level 1) to full alignment (Level 4) – for a number of key business processes.

Communication

The self-assessment questionnaire is an integral part of the plan-do-check review loop.  To effect change and improvement, the answers must be honest.  The results of the assessment and the actions agreed upon should be cascaded through the line using the most suitable medium, e.g. poster, web site, video.

Completing the Questionnaire

The process is straight forward. Take each sub-element, starting with Level 1 and move up the levels only when the statements accurately reflects what is happening in your Asset/s, once you reach a level that does not reflect your level of alignment then tick the box in the previous level.  Only tick a box if the full description is true for the Asset. Once completed, click the "send answers" button at the end of the questionnaire and the results are automatically entered into a database.

Reality Check

In order to obtain the most complete and honest responses, it is recommended that performance figures (LTI’s, leaks, complaints, etc.) are compared with their targets. Where targets are not met, examine the underlying causes to ensure that these shortfalls are reflected in the self-assessment. 

The basis of the ratings should be demonstrable, therefore notes should be retained justifying the decisions. Any recent audits should also be used to substantiate the rating.
