2020 2nd Alert HiPo#62 (OHL) damages to an electrical pole and the conductor, production wells power supply interruption
On 30 Sep 2020, at 0930 hrs., civil crew while removing a temporarily constructed ramp, the wheel loader hit a 33KV I12 pole as it was reversing. The pole and the conductor were damaged and the power supply to production wells got interrupted. No injury were reported to any of the crew members. There was no damage to the wheel loader.
Always ensure appropriate PTW is in place. Always ensure OHL clearance certificate is obtained while working in the vicinity of OHL. Always ensure Banksman is deployed for each plant/equipment. Always ensure site specific hazards are identified in TBT, and ALL workforce have attended the TBT. Always ensure proper communication between operator and banksman.
