2024 Second Alert HiPo#28 Dropped tubing during offloading
At approximately 15:45 at the new location, during the offloading of tubing from a third-party truck, the forklift operator inserted the forks between the tubing layers to support the joints. This arrangement allowed five joints to rest on the forks, while the remaining joints were pushed towards the other side of the truck. Consequently, the tubing crossed over the wooden separation stoppers( Shocks ), which were too short to support the remaining joints. The Rigid Truck stoppers- ( Haulers ) , showing signs of fatigue, could not withstand the pressure exerted by the forklift and failed, resulting in the five joints falling off the truck onto the other side. The area around the truck was effectively managed, with no reported injuries or damage to the tubing
Always Inspect and readjust all side rigid truck stoppers to ensure they meet OEM specifications, avoiding any homemade modifications. Verify that the spacing between tubing joint layers is sufficiently wide to accommodate various types of forks. Install taller wooden separation stoppers to facilitate easier access for the forks. Maintain clear and effective communication between the driver and the forklift operator during the offloading process. Ensure the presence of both a supervisor and a banksman on-site to guide and oversee the offloading operations. Consistently apply SWA (Safe Work Practices) for any uncertainties or doubts during the process. Ensure the presence of both a supervisor and a banksman on-site to guide and oversee the offloading operations.
The plan for offloading tubing at the new location was inadequately discussed There was no established protocol for inspecting these stoppers for defects. The potential risk of engaging the forks in a manner that could push the tubing towards the other side of the truck and over the wooden stoppers was not identified. In section of Stanchions failed to Identify the existing cracks that resulted allowing the GRE Tubing to fall
