During construction of a scaffold platform two single scaffold clips from the same new batch failed while being tightened up. It appears from initial inspection that the materials these are made from are too soft / malleable. One rivet holding the bolt into the main part of the clip has sheared and one has been pulled out. Both have deformed the hole in the main body of the clip.

The clips were manufactured for CAPE Offshore Services in September 2000 and the Batch number is G-784.
