2001/02/16: Learn from a fatal error. Man falling off a canter truck.
The accident happened whilst loading PVC pipe onto the rear of a canter truck. The foreman was standing on the back of the truck instructing two helpers to load the PVC pipe. A pipe spool rolled and knocked him off the truck. He fell to the ground and died of head injuries.
