Lifting of MWD short monel by using crane
The directional Monel measuring 2.61 m length was lifted to the rig floor by using one flat web sling on crane. The monel was kept rested on the rig floor on one edge and still suspended in the crane. When MWD engineer attempted to remove the protector of the monel in hanging position, the monel slipped out of the sling and slid down the v-door to catwalk. No injury to personnel, No damage to tool.
Use lifting caps on both pin and box to lift short monel by using crane with slings and shackles , Or use air winch with lifting cap. Always place equipment on a flat surface.
Inadequate Equipment / the 6 m long sling was inadequate for the job and was incorrectly used. Failure to follow procedure / the monel was rested on one edge on rig floor instead of placing on a flat surface.
