Near Miss - Falling Object
On the rig floor, while picking up liner plug dropping head (after it was made up to a single stand), the bullnose (weighing 0.625 kg.) fell from a height of approx. 9 metres to the rig floor narrowly missing the Assistant Driller.
Check that all components of tools and equipment are tightened and secure PRIOR to lifting. Ensure correct assembly of tools and equipment ¿ using assembly procedures and check sheets.
