Dropped 5" DP Stand
Derrickman dropped stand of 5¿ DP across mast. Floor crew used air hoist to pull it back to monkey board. Air hoist cable too tight, lifted and dragged DP stand out thru V-door where it fell and punched hole in catwalk. No injuries, minor equipment damage.
Toolbox talks are always necessary & useful, Stand must be properly secured before attempting to pull back with air hoist, ensure all personnel cleared from rotary & catwalk areas.
Retrieving dropped drill pipe stand from elevators.
