Ladder Incident
Whilst climbing a ladder the base of the ladder slipped causing the ladder to fall to a lower shelf. The injured person fell to the ground and was hit by a piece of equipment that fell from the lower shelf, resulting in a fracture to his pelvis.
Correct rules/procedures were not followed/communicated.
