Finger Injury
While attempting to remove a lifting sub, using a hammer, a floorman was struck on his right index finger. The hammer glanced off the side of the sub causing a fractured finger.
Ineffective supervision
- The personnel did not hear, did not understand(language) OR did not follow the instructions of the Supervisor, resulting in the wrong tools being used for the job.
