2025 First Alert LTI#04 Floorman suffered fingertip amputation while working on power tong
In preparation for pulling out of hole sucker rods, the floorman was attempting to changes the power tong dies. While doing so he unintentionally operated the tong causing his hand to be caught in between the rotating part of the power tong. This resulted in a crush injury to his left index and middle fingers. The Floorman was evacuated to Haima Hospital, where he was diagnosed with fingertip amputation of his index finger and crush injury to his middle finger.
Do you ensure troubleshooting is conducted by competent maintenance crew? Do you ensure crush points (Line of Fire) are effectively identified & discussed during pre job meetings? Do you ensure to perform dynamic risk review whenever work changes? Always ensure troubleshooting is conducted by competent maintenance crew
The Floorman was unaware his hand was in the crushing point (Line of Fire). There was no proper plan to troubleshoot the Sucker rod Power Tong. The task is a non-routine task.
