2025 First Alert LTI#10 Permit Holder suffered Leg Injury while pulling HDPE Sheet
During the installation of an HDPE liner in a water pit, a wooden anchor piece had been temporarily fastened to the HDPE liner to assist with mechanical pulling. A sudden release of the anchor caused the wooden piece to rebound violently, striking the Permit Holder on the right leg and resulting in a fracture of the bone in his right shin.
Are using the right tools for the task? Are you aware of line of fire risks for the tasks you are working on? Do you ensure to conduct pre-task risk assessments and toolbox talks highlighting potential hazards including Line of Fire? Do you always ensure all the activities are planned well and risk assessed before starting the task?
*Permit Holder was standing in the line of fire during the task. *The crew were using a makeshift set up for this activity. *The risk of this set-up was not anticipated.
