2025 First Alert LTI#14 Driver suffered finger injury
While installing side grills and extensions on a trailer, a driver's left-hand ring finger was caught between the extension and the trailer bed, causing a fracture. The driver received first aid on site and was later treated at Sultan Qaboos Hospital in Muscat.
Do you ask for help when a task is too difficult or unsafe to do alone? How do you make sure everyone on your team knows about pinch point hazards? Do you stop and reassess when something doesn’t feel safe? Do you have process to identify and report hazard related to equipment design? Be aware of the risk when preforming a task and always know your hand position
Driver was working alone handling the side grills and side extensions without assistance. Driver was having difficulties of placing and aligning the side extensions to its position due to design issue. There were gaps in the loading and handling process at the vendor’s location that requires review. The driver was not fully aware of the pinch point hazards.
