The vibrator operator experienced a micro sleep while driving the vibrator, leading to a direct collision with the power line pole Poor supervision – operator was alone near OHPL as he assigned vibrator pusher was not near to monitor or intervene. Risk assessments and MOPO lacked specific controls for OHPL proximity. The operator drove straight toward the pole instead of following the standard VP approach pattern, which typically involves maneuvering to create enough turning space.
