2025 Secound Alert LTI#19 Hand injury while closing radioactive source box
While attempting to secure the radioactive source box, the locking mechanism was partially engaged, requiring the engineer to reposition for better control. During the maneuver, the lock suddenly released, causing the engineer’s right hand to twist and compress the forearm (Ulna bone), resulting in a fracture.
Always assess the risks and the surrounding environment, and apply Stop Work Authority (SWA) in any challenging or unsafe situation. Always seek assistance when necessary—never take unnecessary risks. Always Review Risk Assessment Prior undertaking any activity although simple and routine activities Implement supplementary engineering controls by installing soft-close or hydraulic dampers on RA source box doors across all rigs and facilities to prevent sudden or uncontrolled movements.
Engineer worked from the side of the box, resulting in an awkward posture and added strain. The lid was heavy and exceeded safe one-handed handling limits. Lock design required front access, forcing repositioning and awkward handling. Lock released unexpectedly, causing a sudden load shift and forearm injury. Task involved manual handling without mechanical or dual assistance, contrary to best practices. Box design lacked ergonomic features for safe single-person operation.
