2025 Second Alert LTI#17 Right Hand Ring finger crushed between tongs
Tongs were brought to the rig floor from ground by crane and one of the tongs was suspended with utility winch after setting the hang-off line. When the driller lowered the suspended tong to release the winch, the right hand ring finger of the acting floor man, who was holding tong got crushed between handle of the suspended tong and the arm of the tong which was kept on the rig floor resulting a crush injury.
Do you ensure The rig floor is not congested while Rigging up Tongs? Do you ensure The SOP covers the sequential order of activities when tongs required to be rigged up? Do you restrict unnecessary personnel on the Rig floor while Rigging up tongs?
Power tong was brought to the Rig floor in advance by the Driller, when the operation required to have manual tongs. Power tong occupied considerable space on the Rig floor therefore there was no space for the manual tongs to be Rigged up safely. One of the manual tongs was kept straight under the suspended tongs during rig up. There were three people working on the rig floor during the time of incident
