2025 Second Alert LTI#04 Fingers injury while changing the dies of Sucker Rod Power Tong
While operation was changing sucker rod’s dies from 1” to 1 1/8”. It was observed that the jaw bracket opening was not aligned with tong opening and the Floorman operated the tong slowly to adjust the bracket while his left hand was in pinch point area. When the tong operated, left hand index and middle fingers of the floorman got pinched between the bracket and tong body, resulting crush injury
How do I ensure that all risks—including pinch‑point hazards—are fully identified before starting any task? How do I make sure that all Musta’ed steps are followed carefully and that the required safety mitigations are properly verified? How do I confirm that the proper and required tools and procedures are available and being followed during the task? How prepared am I to immediately stop any unsafe action when I observe it? How do I ensure that the maintenance crew conducting troubleshooting is competent and supported with a proper risk assessment? How do I verify that effective supervision is consistently in place throughout the job? How do I ensure that the pinch‑point register is checked, and that all pinch points are identified and marked on the tools before use?
TBT , JSA ,PTW , Tong Manual and SOP were in place prior to changing the dies. However, specific SOP for SR tong was not available. Hydraulic hose was disconnected however due to stuck jaw bracket and open sliding block HYD hose was connected and power tong operated to align bracket with tong opening. Plan was communicated and supervisor was in rig floor however no one intervened while floorman was positioning his hand in pinch point and rotating the tong. After de-isolation of power tong , sliding door was not closed and hand located in pinch point place. work started without closing the sliding door and removing the hand from the pinch point
