2024 Second Alert HiPo#27 Dropped tubing while RIH
At around 03:42 hrs. the operation was RIH, While picking up the 1st 2^(7/8) joint from the catwalk with an on/off tool to retrieve the packer, the crew used the 3^(1/2) elevator without crossover attached ( it was available in rig floor and disconnected during POOH without proper communication with crew ). As a result, the TBG fell approximately 6mtrs on to the rig floor where the floor man was standing in a safe position at the corner of rig floor away from the LoF.
Driller / AD must ensure to install Crossover with the 2^(7/8) TBG while lifting with 3 ½ elevator. Driller/AD must ensure to communicate clearly and stay attentive to your task. RM/NTP must ensure to check and review the equipment before starting the job. The Crew must ensure to Stop and intervene for any unsafe acts/conditions. The Crew must ensure to follow Musta’ed 7 steps. Always ensure to install Crossover with the 𝟐^(𝟕/𝟖) TBG while lifting with 3 ½ elevator..
NTP and AD failed to ensure that crossover was attached with TBG while lifting with 3 ½” elevator. Traveling block was operated in a moderate speed by AD. Inadequate intervention was there within the crew, prior using the elevator without crossover.
