2026 2nd Alert HiPo#03 MVI collision
The Rig bus driver went to camp to collect lunch, arriving at 11:10 AM. After loading, he departed at 11:50 AM toward the rig site, about 1 km away. During travel, a contracting truck suddenly crossed in front of the bus. Despite immediate braking and horn warning, no response was received, and the bus collided with the truck on the driver’s side at 12.04.58hrs. The incident resulted in bus damage and a minor injury to the driver.
What hazards should you assess before entering a live road from a worksite? How would you verify that it is safe to enter a live traffic route? If you observe a colleague using an unauthorized route, what would you do? How can vehicle marshals and flagmen help prevent vehicle interactions? How can supervisors ensure that agreed controls discussed during TBTs are followed?
The tipper driver entered the live road through an unauthorized access point instead of the approved route. The driver did not fully apply the Stop-Check-Go principle before entering the live traffic area. Traffic control measures at the access point were inadequate, including insufficient signage, speed control, and vehicle marshalling. Planned traffic management controls discussed during the toolbox talk were not effectively implemented or verified in the field. The risk assessment did not adequately identify vehicle interaction hazards and line-of-fire exposure at the road junction.
