2025 HiPo#04 Supporting leg dropped during lifting
The operation was to install the support Legs for the Rig Floor. After installing the support Leg on the Off-Driller side of the Rig Floor, the support Leg on the Driller side was lifted from a Horizontal to a Vertical position using the Crane. A Clamp was attached to the support Leg, with a Web-sling connected to the Clamp. At 20:30 hours, as the support Leg was being lifted into the Vertical position, it disconnected from the Clamp and fell to the ground between the Banksman and the Floorman, who was holding the tagline. No injuries occurred during this incident.
Do not carry out lifting operations until personnel are removed from the Drop zone. Review SOP before starting the job and update the SOP if steps are missing. Ensure that equipment is inspected and fit for purpose. Use only approved Lifting points for lifting. Always adhere to the Musta’ed steps and the discussion from TBT. The securing pins of the Clamp were not secured with Split pins. Generic Lift Plan issued for Lifting the Support Leg. The support Leg was an unstable Load. The Lift Point was not below CG (Center of Gravity). Crew members (Banks-Man and Tag Line Holder) were standing in the Life of Fire. Congested work area in AMAL limits options for Crew positions. Complacent with repetitive activities.
