2024 Second Alert LTI#12/HiPo22 Pipe barrier fell onto helper
on the 11th August, During the skip collection activity scheduled at Rig 74 site, the driver & helper were loading skip boxes on to the truck. Unfortunately, while returning from hooking the chains for the second skip box , the helper was injured when swinging under the barrier, causing it to fall on him and resulting in fractured jaw.
How do you ensure all hazards are identified and communicated before starting and during the task? How do you ensure that employees are away from (LOF) line of fire? How do you confirm an area is safe before entering or passing through it? How do you ensure your crew are complied with SOP? Always ensure to have a clearly defined safe access/egress route before starting work
Helper went under the barricade instead of following safe access arrangements Helper did not request assistance and chose the quickest/unsafe route. Pipe barrier was loose, not properly secured, and unstable. Helper underestimated the risk of passing under the barrier.
