2024 Second Alert LTI#14 Fell while riding tricycle
A HVAC technician was riding a tricycle with a basket to carry tools on the back. He had placed 4-foot step ladders on the basket and wedged them under the seat. As he turned into the workshop yard the ladders slid off and fell in front of the left rear wheel. This, startled the technician who instinctively turned to his left to see what it was and in doing so he inadvertently pulled the handlebars left and this caused the tricycle to lose stability, and it tipped over to the right-hand side. The technician’s right wrist fractured as it contacted the ground

The ladders were not effectively secured to the basket The basket was not designed to effectively hold a set of step ladders This solution had become custom and practice but had not been risk assessed The cyclist was distracted due to anxiety and stress caused by job insecurity
